Release

I agree that by submitting this application that

I will abide by all stipulations stated in the application form and that all statements are true to the best of my knowledge;
I allow audio and video taping and broadcast of all Carlson Horn activities for non-commercial and general promotional use by the Greater Bridgeport Symphony;
I will perform on the March 4, 2012 Finalist Recital and the June 16, 2012 GBS/GBYO Side-by-Side Concert if required by the stipulations of the prize and that if these conditions are not met, I may forfeit my prize and that it will be up to the discretion of the GBS Carlson Horn Committee, GBS Music Director and/or GBS Executive Director to determine if the prize will be awarded to another competitor; and,
That my application will not be considered complete until my application fee of $40 is received by the GBS.

Parents will be required to sign a similar release at the time of competition.
